To open and use these forms you must have Adobe Acrobat Reader installed on your computer. To obtain a free copy of Acrobat Reader click on the Adobe icon below:



TO USE INTERACTIVE FORMS: Once Acrobat Reader is installed, click on the form you want and when it opens you may type in the highlighted areas for text fields. For check boxes, carefully move the hand icon over the box until it becomes a finger pointer and click in the box. A dot will appear in the box which will print with a check mark.
